Immunizations
Immunization Requirements
Texas school and child-care facility immunization requirements are determined by the state legislature and set by the Texas Department of State Health Services, in conjunction with the Texas Education Agency.
Maintaining adequate immunization rates in schools is critical to preventing disease outbreaks and ensuring the health of Texas children.
Students cannot attend school without the appropriate documentation for the required vaccines or a valid exemption. (Texas Administrative Code §97.63)
For students with medical exemptions, a physician's note is required yearly. For students requesting conscience objection of immunization, a written request must be submitted to:
Texas Department of Health
Bureau of Immunization and Pharmacy Support
1100 W. 49th Street Austin, TX 78756
(800) 252-9152
To apply online, please visit Affidavit Request for Exemption from Immunizations for Reasons of Conscience page.
Additional information about immunization exemptions can be found on the Texas Department of State Health Services website.
