If your child will need medication at school or has a medical condition that requires a care plan, be sure to complete the required forms before the first day of school.

Schedule sports physicals before practices begin. Physicals are required for incoming 7th, 9th, and 11th grade students participating in athletics, cheer, or high school marching band, as well as any student needing a new physical. Visit Fairfield Eagles and select Athletic Forms to access Rank One and the required physical forms.
